Belly on smells putrid when I put my finger or fingernail in it. Why?

What to do to remedy? And, why? I am not super fat. I am about 35lbs. overweight. Hard to clean. Hurts if touch too much. Not an infection. Just tight quarters. Hee, hee. : )
